Non-Resident Alien Information Form Instructions

The Non-Resident Alien Information Form must be completed by any Non-Resident who will be working for the University. In addition, any Non-Resident who is receiving a scholarship/fellowship in excess of tuition and believes that the treaty benefit is available to them should also complete this form. The data collected on this form will allow the University to verify eligibility for a tax withholding exemption.

To be able to access this form, please email kaocon02@syr.edu with your SUID number. You will be notified by e-mail when you have been set up as a user.

In order to complete this form you must have your immigration documents, such as, your passport, I-94, I-20, DS-2019, etc readily available. Also some basic biographical information is required.

The form is an electronic form to be completed on the web. Please click on the link below. You will come to a login screen, your User Id is your SUID number (9 digits) and your password is your day (2 digits) and year of birth (4 digits). For example if your birthday is June 7, 1978, your password is 071978. When you have completed all the information for steps 1-5, you will confirm that the information is correct in step 6 and submit the form electronically. You do not need to send anything to the Skytop Office Building at this time.

Any additional form(s) that need to be completed to be able to claim a tax treaty for exemption of tax withholding will be uploaded to the FNIS website if consent is given or mailed to the individual. The individual will then be required to sign the form(s) and return the form(s) to Disbursements before the tax exemption will be allowed.
