Precautionary Measures Bulletin

Directions: This form must be conspicuously posted in the work area prior to the use of any hazardous material without a specific use date listed on the Hazard Identification Form. This notice must be posted as early as possible, but no less than 24 hours before the hazardous material is used. Also fax or courier a copy to the procuring department no less than 24 hours before use.
