Respirator Training and Fit-testing

EHO requires respirator users at the University to attend Respiratory Protection Training and to successfully complete a fit-test before wearing a respirator. EHO's Respirator Training will typically include a fit-test of each attendee at the end of the training.

Respirator Medical Evaluations

All respirator users at the University are required to submit to a medical evaluation and receive medical approval before wearing a respirator.

Supervisor Instructions:

1. Supervisor schedules Respirator Medical Evaluation by calling Industrial Medical Associates (IMA) at (315) 478-1977.
2. Supervisor prints the three forms below for each employee.
3. Supervisor completes the "SU Employee Exposure Info" form.
4. Employee completes the "IMA Respirator Medical Evaluation Questionnaire".
5. Employee brings all three forms to IMA at their scheduled exam time.
