Respirator Assignments
EHO requires respirator users at the University to attend Respiratory
Protection Training and to successfully complete a fit-test before wearing a
respirator. EHO's Respirator Training will typically include a fit-test of each
attendee at the end of the training.

Respirator Medical Evaluations
All respirator users at the University are required to submit to a medical
evaluation and receive medical approval before wearing a respirator.
Supervisor Instructions:
- Supervisor schedules Respirator Medical Evaluation by calling
Industrial Medical Associates (IMA) at (315) 478-1977.
- Supervisor prints the three forms below for each employee.
- Supervisor completes the "SU Employee Exposure Info" form.
- Employee completes the "IMA Respirator Medical Evaluation
Questionnaire".
- Employee brings all three forms to IMA at their scheduled exam time.

Medical Associates (IMA) for a Respirator Medical Evaluation:
